Connect Youth and Family Services offers Adult Support Services to fit the needs of all types of clients and their families. See each type of services below including: Community Support, Mental Health Skill Building (MHSB), and Community Stabilization Services.

*Approval of services are contingent upon prior authorization from Medicaid Managed Care Organizations and/or Comprehensive Services Act.
Community Support

A customized, strength- based, hands on service to assist adults and young adults who are transitioning from foster care.

​Service Provides:​
​

Hands on experience through modeling and repeated task teaching

A comprehensive life skills assessment of the individuals strengths and ways to improve limitations to enhance an individual's independently living skills

A plan to strengthen family and community relationships

Assistance with communication skills, medication compliance

Exploration of housing resources

Training on how to manage personal finances

Healthy meal shopping, preparation and food management

Support with improving personal hygiene

Access and linkage to community resources

Mental Health Skill Building (MHSB)

A task-oriented, individually guided-services for adults who experience difficulties with daily living skills due to their mental health symptoms and diagnoses.

​Service Provides:
​

A comprehensive clinical assessment

Individualized treatment plan highlighting the individuals strengths and interests

Community linkage and ongoing care coordination to develop, maintain and strengthening relationships with providers and natural supports

Social skill development, improves health and wellness
-
Community resources to assist with housing, scheduling and attending appointments, personal hygiene, nutrition, budgeting, money and medication management.
​Criterion for Services:

Are you 21 years or older?
-
Have you been psychotropic prescribed medication within the last 12 months?
-
Have you ever been diagnosed with a mental health disorder?
-
Do you have a prior history of any of the following: psychiatric hospitalization, residential treatment, PACT/ICT services, or temporary detained (TDO) by law enforcement?
-
Would you benefit from support with symptom management, personal hygiene, adherence to psychiatric and medication treatment plan, food preparation, money management and/or development of appropriate social skills?

Community Stabilization Services (CSS)
​
Get Help When Needed...
Services are available 24/7 to provide short-term intensive mental health care to individuals who are currently experiencing a psychiatric crisis.
​
CSS is a bridge service that supports an individual as they are making a transition between certain levels of care when there is a gap in availability of services.

​Service Provides:
Anchored in Hope!
The goal of Community Stabilization Services is to stabilize the individual within their community and support the individual and/or their existing support system during the periods 1) between an initial Mobile Crisis Response and entry into an established follow-up service at the appropriate level of care, 2) as a transitional step-down from a higher level of care if the next level of care service is identified but not immediately available for access or 3) as a diversion to a higher level of care.

Criterion for Services...

Virginia Medicaid eligible.
-
Adults 18 and older who are in need of ongoing support and services.
-
Experiencing difficulty in establishing and maintaining normal interpersonal relationships to a degree that one is at high risk of psychiatric hospitalization, homelessness or isolation from social supports.
-
Experiencing difficulty in activities of daily living, such as hygiene, food preparation or managing finances to the degree that health or safety in jeopardized.
-
Inappropriate behavior that necessitate immediate interventions by mental health, social services or the judicial system.
-
Difficulty in cognitive ability which demonstrates an inability to recognize personal danger or recognize significantly inappropriate social behavior.
